One claim record
A useful claim file separates what happened, what the policy says and what was paid. Keep originals or clear copies and note when each item was received or sent.

When a decision is unclear
Ask for the coverage, exclusion, limit or condition used in the decision.
Correct factual errors with documents rather than relying on a phone conversation alone.
Save the explanation with the date, sender, claim number and any next-step deadline.
Ask about the insurer's review process and your state insurance department's consumer services.
